What is adult day care in South Carolina?
Adult day care is a DPH-licensed daytime program where older adults or adults with disabilities spend the day with supervision, meals, activities and often health monitoring, returning home at night; each license sets a maximum number of participants.
DPH licenses 17 adult day care facilities in the four counties, with licensed capacity for 1,058 participants in all. Centers vary from small programs to large ones licensed for more than 100 participants.
How much does adult day care cost?
CareScout's 2025 South Carolina median for adult day health care five days a week is $18,720 a year, about $1,560 a month, the least expensive paid care option in the survey.
Many families use two or three days a week, which lowers the cost proportionally.
Who pays for adult day care?
Families pay privately at most centers; the Community Choices waiver covers adult day health care for eligible Medicaid members; PACE participants attend PACE's own licensed center in Greenville; and some long-term care insurance policies pay.
Medicare does not pay for adult day care. Ask each center about transportation, nursing on site and dementia care.
How do I choose an adult day center?
Visit on an ordinary weekday, stay through a meal, and ask about hours, transportation, staff-to-participant ratios, whether a nurse is on site, how medications are given, how the center handles dementia-related behaviors, and how it communicates with families.
Check the center's DPH license, which shows its licensed participant capacity, on our adult day care roster or DPH's Find a Facility map. Ask whether the center is enrolled as a Community Choices provider if Medicaid may help pay.
Is adult day care good for people with dementia?
Often, yes: structured activity, social contact and a predictable routine help many people with mild to moderate dementia, and the program gives family caregivers a reliable break; ask how the center handles wandering, agitation and toileting.
The first weeks can be hard as the person adjusts, so centers often suggest starting with two or three days a week. Some people with dementia do better with care at home if any change of setting distresses them.
