Pickens County had 26,668 residents 65 and older in 2025 Census estimates. DPH licenses 9 senior-serving CRCFs and 6 nursing homes there, concentrated in Easley and Clemson, with retirement campuses that combine levels of care.
A county with two centers
Pickens County runs from the Greenville County line at Easley and Powdersville west to Clemson and north into the Blue Ridge foothills toward Pickens and Six Mile. Easley is its largest city, with about 27,319 residents in 2025 estimates; Clemson, home to Clemson University, about 18,941. Clemson's student population gives the county the youngest median age of our four counties, 36.6, but 19.2% of residents are 65 or older.
Clemson also draws retirees, including returning alumni and retired faculty, which helps explain its cluster of retirement and assisted living communities.

Retirement campuses
Two Pickens County campuses combine assisted living with nursing care at one address: the Presbyterian Communities campus in Easley holds both a CRCF and a nursing home license, and the Clemson Area Retirement Center has a licensed nursing home health care center alongside its residential options. On such a campus a resident can move between levels of care without leaving familiar people and surroundings.
Continuing care campuses often involve entrance fees and contracts that differ from month-to-month assisted living. Read the contract carefully, ideally with an attorney, before committing.
Enforcement on the record
DPH's March 2026 enforcement report included an order involving the operator of Easley Retirement Center, a CRCF in Easley; its facility page summarizes the entry. A single order is a reason to ask what happened and what changed, not a verdict on the building. Most Pickens County facilities do not appear in the reports from January 2025 onward.
Dementia care in Pickens County
Several Pickens County CRCFs carry an Alzheimer's special care unit flag on DPH's data, including buildings in Easley and Clemson, and the Presbyterian Communities nursing home in Easley also carries an Alzheimer's unit flag. For a parent whose dementia is progressing, a building with both standard assisted living and a secured unit can spare a second move. Ask each one for the written disclosure the Alzheimer's Special Care Disclosure Act requires and how many staff are on the unit at night.
Families in the northern part of the county should weigh the drive carefully: visits are what keep a person with dementia connected, and a building 15 minutes closer may matter more than any feature list.
Paying for care in Pickens County
Costs follow the state pattern: South Carolina's 2025 median for assisted living is about $5,350 a month and for a semi-private nursing home room about $9,034. Retirement campuses with entrance fees price differently, so compare the total cost over several years rather than the monthly fee alone. For residents with very low income, ask which CRCFs participate in Optional State Supplementation.
For care at home, the Community Choices waiver and PACE-Upstate both serve the county, and DPH's data lists licensed in-home care providers and adult day centers in Easley, Liberty and Pickens. Adult day care is the least expensive paid option for a parent who is safe at night but alone during the day.
Hospitals, PACE and help
Prisma Health Baptist Easley Hospital and AnMed Cannon in Pickens are the county's hospitals, and Greenville's and Anderson's larger hospitals are within reach. Prisma Health SeniorCare PACE-Upstate serves Pickens County. The Appalachian Council of Governments Area Agency on Aging (864-242-9733) handles information and referral, the regional ombudsman program and I-CARE Medicare counseling for the county.
Families in Six Mile, Central or Pickens often compare a local option with Easley or Clemson. As always, the person who will visit most should weigh the drive.
