What is in-home care in South Carolina?
In-home care means help delivered in a person's home, either non-medical personal care and companionship from a DPH-licensed in-home care provider, or skilled nursing and therapy from a DPH-licensed home health agency under a doctor's order.
DPH's data lists 295 licensed in-home care providers with offices in Greenville, Anderson, Pickens and Laurens counties, 227 of them in Greenville County. Families often combine both after a hospital stay.
How much does in-home care cost?
CareScout's 2025 South Carolina median for a non-medical home care worker is about $31 an hour, or $5,982 a month at 44 hours a week; Medicare-covered home health visits cost an eligible patient nothing.
Agencies often set minimum shift lengths. Around-the-clock care at home costs several times the assisted living median.
Who pays for in-home care in South Carolina?
Medicare pays for skilled home health for homebound patients; the Community Choices waiver pays for personal care, companion care and respite for Medicaid-eligible adults at nursing facility level of care; PACE covers home care for its participants; and families, long-term care insurance and VA benefits pay for the rest.
Start with the Area Agency on Aging to screen for programs before paying privately.
How do I choose an in-home care agency in the Upstate?
Confirm the agency's DPH in-home care license, then ask about minimum shift length, how aides are screened and trained, whether aides are employees or contractors, who covers when an aide calls out, how care plans are updated, and whether the agency accepts Community Choices clients or long-term care insurance.
Ask for rates in writing, including weekend, holiday and overnight rates. Agencies with offices near your parent often staff short or early-morning shifts more reliably. Our in-home care roster lists every licensed provider by city.
When is in-home care no longer enough?
When help is needed at unpredictable hours, especially overnight, when falls or missed medications keep happening despite paid help, when a parent is isolated, or when paid hours climb past about 40 a week, a CRCF's around-the-clock staffing often becomes safer and less costly.
At CareScout's 2025 South Carolina medians, 40 hours a week of home care costs about the same as the assisted living median, and 24-hour home care costs several times more. Track incidents for a few weeks before deciding.
