TL;DR

Aid and Attendance raises a veteran's or surviving spouse's maximum pension rate; VA pays the difference between that rate and countable income after care costs are subtracted. For a veteran paying South Carolina's median assisted living cost, care costs can wipe out countable income, producing a pension near the full $2,424 a month for a single veteran.

How the benefit works

VA's Veterans Pension and Survivors Pension are needs-based benefits for wartime veterans and their surviving spouses with limited income and assets. Aid and Attendance is an increase for those who need help with daily activities, are bedridden, live in a nursing home or have limited eyesight. It is not a separate check. It raises the Maximum Annual Pension Rate (MAPR), and VA pays the difference between the MAPR and the household's countable income.

The key is that unreimbursed medical and care expenses, including assisted living and home care costs related to the need for care, are subtracted from income before the comparison. That is why a veteran whose income looks too high can still receive a pension once care costs are counted.

The 2026 rate table

These maximum annual rates took effect December 1, 2025:

SituationMaximum annual rateAbout per month
Veteran with no dependents, Aid and Attendance$29,093$2,424
Veteran with one dependent, Aid and Attendance$34,488$2,874
Two veterans married to each other, both A&A$46,143$3,845
Surviving spouse, no dependents, Aid and Attendance$18,697$1,558
Veteran with no dependents, Housebound$21,313$1,776

VA's net worth limit for the same period is $163,699, counting assets plus annual income, with the primary home excluded.

A worked example

Consider a widowed veteran living in Simpsonville with $2,100 a month in Social Security and a small pension of $300, so $2,400 a month or $28,800 a year. He moves into a CRCF charging $5,350 a month, South Carolina's 2025 median, and needs help with bathing and dressing.

Without care costs, his $28,800 income is close to the $29,093 Aid and Attendance maximum for a veteran with no dependents, so his pension would be small. With care costs, VA subtracts the unreimbursed assisted living expense, roughly $64,200 a year, less a small deductible amount VA applies, from his income. His countable income falls to zero, and his pension can approach the full maximum of about $2,424 a month.

That still leaves a gap: his income plus pension is about $4,824 a month against a $5,350 bill. Aid and Attendance rarely covers everything, but it can stretch savings by years. Exact amounts depend on VA's calculation, so treat this as an illustration, not an estimate of any particular claim.

What a surviving spouse can expect

For a surviving spouse with no dependents, the Aid and Attendance maximum is $18,697 a year, about $1,558 a month. The same care-cost calculation applies. Many widows of Korea and Vietnam era veterans in the Upstate qualify and never apply, often because they do not know the benefit extends to surviving spouses.

Documenting care costs

VA decides claims on paperwork, so the quality of the documentation shapes the result. Ask the CRCF for a letter on its letterhead stating the monthly charge and that the resident receives help with specific daily activities such as bathing, dressing or medications. For home care, keep invoices from a licensed in-home care provider. For a nursing home, the monthly statement is usually enough.

Report changes in income or care costs to VA promptly. Overpayments must be repaid, and an unexpected debt to VA is a hard thing for a family to absorb in the middle of a parent's care.

Housebound benefits and home care

Not every veteran who needs help moves into assisted living. Aid and Attendance can also be paid for veterans living at home who need help with daily activities, and unreimbursed home care costs from a licensed in-home care provider reduce countable income the same way. A separate, lower Housebound rate, $21,313 a year for a veteran with no dependents under the current table, applies to veterans substantially confined to the home.

For a veteran using 20 or 30 hours a week of home care at South Carolina's median of about $31 an hour, the care cost alone can be $2,700 to $4,000 a month, often enough to make a meaningful pension payable.

How to apply without paying anyone

Every South Carolina county has a county veterans affairs office whose accredited service officers help with pension claims free of charge, including Greenville, Anderson, Pickens and Laurens counties. Bring the veteran's discharge papers (DD-214), marriage and death certificates as needed, income and asset records, the facility's statement of monthly charges and care services, and a physician's statement of the need for help with daily activities.

VA applies a three-year look-back to assets transferred to get under the net worth limit, with penalties of up to five years. Be cautious of anyone who charges to reposition assets for VA purposes. For veterans who need nursing home care, the Richard M. Campbell Veterans Nursing Home in Anderson is a state veterans home with its own admission process.

Who helps file a claim for free?

Each county's veterans affairs office, through accredited service officers.
