TL;DR

Every South Carolina CRCF must have at least one staff member per eight residents (or fraction of eight) during peak hours and one per 30 during non-peak hours, with someone awake and dressed at all times. Those are floors. The question for families is how far above them a building staffs, especially at night and on secured units.

What the regulation says

Section 503.B of Regulation 60-84 sets the minimums for each building: at least one staff member or direct-care volunteer for each eight residents or fraction thereof on duty during all periods of peak hours, and at least one for each 30 residents or fraction thereof during non-peak hours. A staff member must be awake and dressed at all times. In buildings licensed for more than 10 beds and built on more than one floor, a staff member must be available on each floor whenever residents are there. The facility must keep records proving it meets both ratios.

South Carolina is one of a minority of states that set a numeric ratio for assisted living at all, which gives families a concrete starting point.

What the minimum looks like by building size

The phrase or fraction thereof means the count rounds up. Here is the arithmetic for typical Upstate building sizes, assuming every bed is full:

ResidentsMinimum staff at peak hours (1 per 8)Minimum staff off-peak (1 per 30)
1021
1621
2431
4462
5272
6483
90123
110144

The median senior-serving CRCF in our four counties has 64 licensed beds, so a full median building needs at least 8 staff at peak times and 3 overnight under the rule.

Why the minimum is not the whole story

The ratio counts heads, not needs. A building where many residents use wheelchairs, need two people to transfer, or have dementia will struggle at the minimum. The regulation itself says staffing must be sufficient to provide supervision, care and basic services for all residents, which can require more than the numeric floor.

The ratio also applies to the building as a whole. South Carolina sets no separate ratio for an Alzheimer's special care unit, so a building could meet its overnight minimum with staff who are covering both the secured area and the rest of the building. Ask how staff are assigned on the unit.

What peak hours means

The regulation does not define peak hours by the clock. In practice they are the stretches when residents need the most help: waking, bathing and dressing in the morning, meals, and the evening routine. Ask each facility when it switches from peak to non-peak staffing. A building that drops to its overnight number at 7 p.m. will feel very different at bedtime from one that keeps peak staffing until 10.

How the ratio interacts with medications

In a South Carolina CRCF, trained staff who are not nurses may give residents their routine medications, acting in what the regulation calls a surrogate family role. The same staff counted toward the ratio may therefore be running a medication pass for many residents at breakfast, which is also a peak period for bathing and dressing. Ask how the building schedules medication passes and whether a dedicated medication aide covers them.

Regulation 60-84 also bars staff from giving sliding-scale insulin and limits staff injections to diabetes and anaphylaxis under medical protocol. A resident whose medications need frequent adjustment is outside what a CRCF may handle, regardless of staffing.

Small homes and the overnight shift

In the smallest CRCFs, the arithmetic can come down to one person. A 10-bed residential home needs two staff at peak hours under the one-per-eight rule but only one overnight under the one-per-30 rule. That person must be awake and dressed, and must be able to respond to every resident's needs. Ask a small home what happens if two residents need help at once at night, who the on-call backup is, and how quickly that person can arrive.

Small homes can be excellent precisely because staff know every resident well. The overnight plan is simply the place to probe.

Questions to ask on a tour

Ask to see the staffing schedule for a typical weekday and weekend. Ask how many staff are on duty right now and how many residents are in the building. Ask how many residents need two-person assistance. Ask how often shifts are filled by agency staff and how the building covers a call-out at 2 a.m. For a secured unit, ask how many staff are physically on the unit at night. Because the facility must document that it meets the ratios, these answers should exist on paper.

Frequently asked questions

What is the assisted living staffing ratio in South Carolina?

One staff member per eight residents or fraction thereof during peak hours and one per 30 off-peak, with someone awake and dressed at all times (Regulation 60-84, Section 503.B).

Does South Carolina require a nurse in assisted living?

No. Regulation 60-84 does not require a licensed nurse on duty in a CRCF; trained staff may give routine medications.

Is there a separate ratio for memory care?

No. The building-wide CRCF minimum applies to Alzheimer's special care units.

Do facilities have to prove they meet the ratio?

Yes. Regulation 60-84 requires documentation showing the peak and non-peak minimums are met.
