Laurens County's 20.9% share of residents 65 and older is the highest of our four counties, and its licensed care sits mainly in Laurens and Clinton. It is served by Upper Savannah COG rather than the Appalachian COG, and the Upstate PACE program does not cover it.
How old is Laurens County?
Census Bureau estimates for July 1, 2025 put Laurens County's population at 71,848, with 15,022 residents 65 and older and 1,427 aged 85 and older. Its median age of 41.6 is the highest of the four counties. The cities of Laurens and Clinton each had a little under 10,000 residents, and the county also includes Gray Court and part of Fountain Inn.
That combination, an older population spread across small towns, shapes the senior care picture: fewer large purpose-built communities, more reliance on long-established nonprofit campuses, and a real need for help at home.

Campuses that offer more than one level of care
Two Laurens County campuses hold both a CRCF license and a nursing home license at the same address: Presbyterian Communities of South Carolina in Clinton and the Martha Franks retirement community in Laurens. On a campus like that a resident can move from assisted living to nursing care, or go to rehab and return, without leaving familiar surroundings. That matters especially for couples when one spouse needs more care than the other.
NHC operates nursing homes in both Laurens and Clinton, and Prisma Health Laurens County Hospital in Clinton is the county's hospital.
Which agency helps Laurens County seniors?
Laurens County belongs to the Upper Savannah Council of Governments aging region, based in Greenwood, along with Abbeville, Edgefield, Greenwood, McCormick and Saluda counties. Its Area Agency on Aging and Aging and Disability Resource Center handles information and referral ((864) 941-8077), assessments for in-home services, family caregiver support, SHIP Medicare counseling and the regional long-term care ombudsman program ((864) 941-8070).
Families who have dealt with the Appalachian COG for a relative in Greenville County are sometimes surprised to be redirected; it is simply a different region. State programs, including DPH licensing, Medicaid, Optional State Supplementation and the Community Choices waiver, work the same way in Laurens as everywhere else.
Help at home in Laurens County
Because so much of Laurens County is rural, staying at home with help is often the first choice. DPH's data lists licensed in-home care providers with Laurens County offices, plus home health agencies in Clinton, and agencies based in Greenville County also serve many Laurens addresses. Ask any agency how far it will send aides and whether it has a minimum shift length, which matters more when travel time is long.
For Medicaid-eligible adults at nursing facility level of care, the Community Choices waiver can pay for personal care, companion care, adult day health care and respite at home. Referrals go to SCDHHS Community Long Term Care at (888) 971-1637, and Upper Savannah COG can help families start the process.
What is missing, and where do families look?
Prisma Health SeniorCare PACE-Upstate serves Greenville, Pickens and Anderson counties but not Laurens. Laurens County also has fewer Alzheimer's special care units than its neighbors, so families needing memory care often look north to Fountain Inn and Simpsonville, a 20- to 30-minute drive up I-385, or to Greenwood.
Before looking outside the county, compare what a local option offers against the cost of distance. Frequent visits from family and old friends often matter more to an older adult's well-being than a newer building farther away.
