TL;DR

DPH's roster lists 7 senior-serving CRCFs with Simpsonville, Fountain Inn or Mauldin addresses, most of them large buildings with Alzheimer's special care units. Size, the Alzheimer's flag, the licensee and the enforcement record are the differences the state's data can show; staffing, price and atmosphere you have to see for yourself.

Why so much assisted living in southern Greenville County?

Simpsonville and Five Forks grew quickly over two decades, and assisted living followed the rooftops and the I-385 corridor. Simpsonville's population rose from about 23,400 in the 2020 Census base to about 28,500 in 2025 estimates, and Fountain Inn grew even faster in percentage terms. Prisma Health Hillcrest Hospital in Simpsonville and the hospitals in Greenville are within reach.

For a family that lives on the south side of the county, that means a real choice within a short drive, which is the best position to be in.

How to read the differences

Licensed beds tell you scale. A 90-bed building usually has more activities and a fuller dining program; a smaller one may feel more like a household. Neither is better in general; it depends on the person.

The Alzheimer's flag tells you whether the building reports a secured dementia program. If memory care may be needed later, a building with both standard assisted living and a flagged unit can avoid a second move.

The licensee's home state tells you who is ultimately responsible. Many larger buildings here are operated by companies based outside South Carolina. That is common and not a problem by itself, but it is worth asking who manages the building day to day and who a family calls above the administrator.

DPH orders are rare and significant. In the April 2026 report, for example, DPH listed an order involving the operator of Pearl at Five Forks; its facility page summarizes what the report says. One order does not define a building, but it is a reason to ask what changed.

What the state data cannot tell you

South Carolina does not publish CRCF inspection reports online or rate assisted living. You will not find staffing levels, prices or resident satisfaction in any public dataset. Those come from the tour and from documents the facility should hand you: the admission agreement, the latest DPH inspection report with its plan of correction, the staffing schedule for peak and overnight hours, and for flagged units, the written Alzheimer's special care disclosure.

Ask each building the same questions so the answers are comparable, and visit at least once in the evening.

A short checklist for comparing buildings

Put the same five questions to every building on your list. What is the total monthly cost for my parent's care level, in writing? How many staff are on duty at 7 a.m., 7 p.m. and 2 a.m.? When did DPH last inspect, and may I see the report and plan of correction? What would require my parent to move out? Does the building take OSS or Community Choices Residential Personal Care?

Write the answers in one table. Differences that are invisible on a tour, such as a care fee that jumps at the next level or an overnight staffing number at the regulatory minimum, often become obvious side by side.

When the nearest hospital matters

For residents with heart failure, COPD or diabetes, the distance to an emergency department matters. Prisma Health Hillcrest Hospital in Simpsonville is the closest for most buildings in the area, and Greenville Memorial and St. Francis Downtown are farther north. Each facility page on our site lists the two nearest general acute-care hospitals on DPH's roster by straight-line distance.

Ask each building which hospital ambulances usually take residents to, and whether staff accompany residents or send a transfer packet.

Paying for it

CareScout's 2025 South Carolina median for assisted living is about $5,350 a month, and newer buildings in fast-growing suburbs often price above the median. Ask whether each building participates in Optional State Supplementation or Community Choices Residential Personal Care. Most large private-pay communities do not participate in OSS, which matters for a resident whose savings may run out.
